Death of a Covered Dependent
If your spouse, certified domestic partner or child dies, you should notify Human Resources to make appropriate changes to your employee record and to your benefits. You must provide a copy of the death certificate to Human Resources within 30 days of the death in order to make changes to your benefits.

Health, Dental, Vision and Accidental Death & Dismemberment Coverage
Changes must be made within 30 days of the death.
- You should change the level of your health, dental, vision and/or accidental death & dismemberment coverage, if applicable.
Flexible Spending Accounts (FSAs)
Changes must be made within 30 days of the death.
- Due to the death in your family, you may want to begin participating in an FSA, or increase or decrease the contributions to your existing FSA.
- You may obtain the FSA Enrollment Form from HR Express, 2525 West End Avenue, second floor or from the Forms and Documents page.
Retirement Benefits
Changes should be made as soon as possible after the death.
- If your spouse is the deceased, you must update your marital status with your retirement vendor, so your beneficiary designation can be updated.
Life Insurance
Change can be made anytime during the year.
- You may need to change your beneficiary designation.
