    If you are the surviving spouse, domestic partner or dependent of a Vanderbilt employee, you should notify your loved one’s department administrator and the Benefits Office in Human Resources.

    Action Items Checklist:

    Notify Department Supervisor.

      If unsure how to contact the department, you may start with the Vanderbilt University operator by calling 615.322.7311 or the Vanderbilt University Medical Center operator by calling 615.322.5000.
       

    Notify the Benefits Office.

      Please call 615.322.8977 to inform the HR representative of the death.
       
    Make decision about continuing health and dental insurance coverage through COBRA.
      Go to the Forms and Documents page and find the Continuation of Benefits Form—COBRA in the list. Complete the form and mail to: COBRA Administrator, VU Station B #357700, 2301 Vanderbilt Place, Nashville, TN 37235-7700 within 60 days of the death.

    Health and/or Dental Coverage

    Changes must be made within 60 days of the death.

    Flexible Spending Accounts (FSAs)

    If your loved one was enrolled in an FSA for health or dependent care expense reimbursement, the enrollment ends on the day that the death occurred. However, survivors have until the FSA submission deadline to submit claims for expenses incurred prior to the death.

    Retirement Benefits and Life Insurance

    Contact the HR representative in the Benefits Office by calling 615.322.7311 as soon as possible after the death.
