My Child Is No Longer an Eligible Dependent
If your child gets married, turns 25, or no longer depends on you for 70% or more of his/her financial support, he/she is no longer eligible to be on our health, dental and vision plans and you must remove the child from your coverage. Complete the Notification of Family Status Change Form and submit it to the Benefits Office, VU Station B #357700, 2301 Vanderbilt Place, Nashville, TN 37235-7700 within 30 days of the eligibility change.
