Disease Management
Coverage under each of the health options (Standard, Advantage P and HealthFund) includes free participation in a disease management program administered by ActiveHealth.
The goal of the ActiveHealth disease management program is to improve employee health and control costs.
A Two-part Program
The first part of the program reviews the care that you receive to identify any potential problems. An example might be that if a participant has two doctors prescribing medications that shouldn’t be taken at the same time, something called a “Care Consideration” will be faxed to the doctors so they can act upon it.
The second part of the program identifies health plan participants with specific chronic health conditions, such as diabetes or heart disease, and automatically notifies them of the program to help monitor their condition, provide online tools, and offer a nurse phone line. If you have one of the chronic conditions monitored by ActiveHealth you will automatically contacted about the chronic conditions portion of the program and you may opt-out. Vanderbilt encourages participation to provide fullest support available for your health.
The disease management program is included in your health plan enrollment and is provided to you at no cost.
