Same-Gender Domestic Partner Certification Checklist
Can you answer "Yes" to and provide proof of each of the following?

| My partner and I are at least 21 years of age. | ||
| My partner and I are single, not legally married to anyone else. | ||
| My partner and I are currently in a committed relationship and have been in such for six months or more. | ||
| My partner and I reside in a common household and share responsibility for the household. | ||
| I have not received nor requested Domestic Partner Certification with Vanderbilt within the last 12 months. | ||
| If my partner has a dependent child(ren) who I plan to add to my health or dental coverage, that child(ren) meets the Internal Revenue Service definition of a dependent. |
If you answer "Yes" to each of these questions, the next step to certify your same-gender domestic partner is to contact the Director of Benefits' office to make an appointment.
