
My Spouse’s/Partner’s Coverage Changes
If an event causes your spouse/certified domestic partner to gain or lose health, dental and/or vision coverage, or their employer's coverage changes significantly during their company's Open Enrollment period, you may want to add or delete them from Vanderbilt's coverage, or enroll in our plan(s) if you previously waived coverage. If you want to change the coverage for your spouse/partner, you must complete the Notification of Family Status Change Form and official documentation describing the change, and submit the completed forms to the Benefits Office, VU Station B #357700, 2301 Vanderbilt Place, Nashville, TN 37235-7700 within 30 days of the event that triggered the coverage change (that is not necessarily the same date as the coverage change itself). You must provide supporting documentation in order to make changes to your coverage.
