Forms used to Classify a New Position or Reclass an existing position
Position Management Form (PMF)-used to create new position or make changes to established positions.
This form indicates home department, title, pay group, budget and cost center information. This is the form used to obtain budget approval for new positions or changes to existing positions.
Position Description Questionnaire (PDQ)-used to describe job duties for a new position or changes in established position.
This form is used by the Office of Compensation to review job duties and properly classify the position. It indicates job duties, percentage of time spent on tasks, budgetary responsibilities, supervisory responsibilities, and level of complexity and independence.
Position Description Questionnaire-EZ ( PDQ-EZ) is an alternative to the PDQ, and can be used anytime a department prefers to submit job duties attached in an alternative format, ie, Word document, etc.
Personnel Action Form (PAF)-used to add new hires to payroll or to implement changes for current staff if changes in salary, hours, home department, reclass, etc. This form indicates incumbent information, and includes information such as standard hours and compensation. This form is submitted after the classification/reclass process is completed to implement any changes for incumbent or to add a new hire to payroll.
Where to send form(s):
Step 1-PMF should be signed by appropriate budgetary authority in respective school, division, or department.
Step 2-PMF (and applicable forms above) should be sent to:
VUMC-Medical Payroll
University Central-HR Records Management
Provost areas-Associate Provost Office
Step 3-Above areas will forward PMF/PDQ to Compensation or Recruitment, as appropriate.
Step 4-Forward PAF through steps 1 & 2 above to add new hire to payroll or to implement position reclass recommendation for an incumbent. Please attach classification recommendation report received from Compensation to the PAF.
