Marriage

You and your spouse will want to evaluate your benefit options to be sure your coverage makes sense for both of you and for any children that might be in your new family. Marriage is considered a qualifying life event, which allows you to declare a Family Status Change and make changes to your benefits during the plan year. Changes must be made within 30 days of the marriage and  consistent with and as a result of the marriage.

Action Items Checklist:
Change name (if applicable) and marital status.
  An employee must submit the Personal Information Change Form and a copy of his/her new social security card to initiate a name change, with a revised W-4 Form for 2016. Forms can be mailed to: HRRM, PMB #407718, 2301 Vanderbilt Place, Nashville, TN 37240-7718. (Changes to your W-4 cannot be made online in C2HR or by fax. The IRS requires an original signature to make this change.) See Change to Records for more information.
   

Change tax withholding status (if applicable).

  Go to the Tools and Forms page and find W-4 in the list. Complete the form and mail to HR Records, PMB #407718, 2301 Vanderbilt Place, Nashville, TN 37240-7718. (Changes to your W-4 cannot be made online in C2HR or by fax. The IRS requires an original signature to make this change.)
   
Checkbox Update your marital status using the Personal Information Change form .
   
Make changes to benefits (if applicable). 
  See Family Status Change for more information. If you are currently covering your same-sex domestic partner, and you are legally married, you need to contact the Human Resources and inform them your domestic partner needs to be updated to spouse and provide the date of the marriage.

Health and/or Dental Coverage

Changes must be made within 30 days of the marriage.

  • If you wish to add your spouse and newly eligible children to your coverage; or drop your coverage if your spouse is going to cover you under his or her plan, you must declare a Life Change within 30 days of the status change. See Family Status Change for more information.

Flexible Spending Accounts (FSAs)

Changes must be made within 30 days of the marriage.

  • If you anticipate changes to your health care or dependent care expenses in the upcoming months due to your change in marital status, you may want to begin participating in a FSA, or increase or decrease the contributions to your existing FSA.

Retirement Benefits

Changes should be made as soon as possible after the marriage.

  • Contact your investment company to update your beneficiary designation.

Life Insurance

Changes can be made anytime during the year.

    • You may want to change your beneficiary designation. Requests to change your coverage level are allowed anytime during the year.  See Changing Life Insurance for details on how to apply.
