- Step 1- Medical Leave Request form. Complete the online medical leave request form.
- Step 2- The medical certification from will be provided to the employee by Human Resources along with the eligibility notice.
- Step 3- People Experience will provide the employee and supervisor with the approval letter once the medical determination and eligibility have been reviewed.
- Step 4- Managers please track the employee's FMLA utilization in Oracle.
- Step 5- Release to Return to Work
