If you’re just starting to research your options, enrolling for the first time, or re-enrolling after many years, Fidelity’s licensed experts can help you understand your options, costs, and deadlines. The licensed Medicare advisors at Fidelity do not work on commission, so you can trust them to be knowledgeable and to answer your questions honestly without trying to steer you.

This service is not just limited to you or your spouse. If you are helping a parent, loved one, or friend navigate their Medicare journey, the Medicare advisors at Fidelity are available to help.

Medicare Enrollment – Form L564 (Request for Employment Information)

If you did not enroll in Medicare when you were initially eligible because you have or had coverage through the Vanderbilt health plan, you will need to show that you had continuous coverage in order to avoid a lifetime premium penalty.

During your Special Enrollment Period, which is an eight-month period that begins the month after your group health plan coverage ends, you will need to enroll and submit Medicare form L564 Request for Employment Information, to show the Social Security office that you held continuous coverage.

To request a form, please email The Benefits Team at Vanderbilt at benefits@vanderbilt.edu. We will complete Sections A and B, sign, and return to form to you. You do not need to send a form to the Benefits team.
